WHICH INCISION?

Effect of Incision Choice on Outcomes in Primary Breast Augmentation

If you are thinking about having breast augmentation, one of the factors that needs consideration is the site of the incision (where the cut in the skin will be created to place the implant through).

The answer? Not with magic. But with consideration for each anatomical component of the breast, and the state at where they are compared to where they ideally should be. To break it up into simple terms to understand, let’s consider three components of the sagging breast:

1. The nipple

“Droopiness” of the breast is often termed breast ptosis. This relates to and is classified by the position of the nipple in relation to the breast it lies on. In the ideal breast, the nipple should be pointing forward, and positioned above the inframammary fold (the breast crease). With age and especially after breast feeding, the breast substance is reduced, and the supporting tissues of the breast begin to weaken, causing the nipple to sag and move south. One part of a breast lift (mastopexy) is to reposition the nipple back up to where it should be.

2. The breast tissue (parenchyma)

The size of the breast can wax and wane, influenced by weight changes, pregnancy, as well as normal hormonal variations throughout the menstrual cycle. Some women who have lost significant weight find that they’ve lost substance to their breasts. Post breast-feeding women experience a similar deflation of their breasts. 3. The breast skin

The key factor here is to consider the elasticity of the breast skin. To help understand the correlation between skin elasticity and sagginess, here are two analogies:

Analogy 1: consider a brand new rubber balloon inflated and expanded with air. When a little bit of the air is released, the elastic balloon will reduce in size, but at all times maintain the general shape of the inflated balloon. Only when the air is nearly all out will the shape of the inflated balloon be lost. This is analogous to a youthful breast that for whatever reason has its volume reduced. When the volume reduction is minimal to moderate, the elasticity of the breast skin may be able to cope with maintaining the shape of the breast. Analogy 2: consider a plastic bag similarly filled with air. When the air is slowly released from this, the inelastic nature of the plastic bag does not allow for uniform shrinkage of the bag, and the bag becomes loose and out of shape immediately. This is analogous to the skin of the ageing, post-menopausal breast. The breast has lost its elasticity and ability to expand and retract according to the volume of the breast. Breast Implants or Breast Lift?

Of course, to be entirely sure, a complete assessment by a fully qualified plastic surgeon experienced in breast enhancement surgery is necessary. However, there are a few tips that patients can apply to get a fairly close idea as to which procedure is required to give them the result they are looking for.

The “look in the mirror” assessment:

Patients can gain a quick assessment just by looking at the mirror (with arms down by your side) or at photographs taken of them.

Try to avoid “selfies” of breasts as these are often taken at incorrect angles and distances, rendering them distorted and inaccurate.

When you look at the mirror, make a note of where you inframammary fold or breast crease is. The folds on each side may be at different levels, and this factor in breast asymmetry is common and can be considered normal.

Once the above questions have been answered, we have most of the necessary information required to determine which is the best operation to perform; this however has to be balanced with what result we’re aiming for, as there are sometimes limitations caused by the patient’s anatomy.

In general, if your nipple and most of your breast tissue lies above the breast fold, then you should get a very good and long-lasting result with implants alone. On the flip side, if your nipple and most of your breast tissue lies below the fold, then you will NOT be able to get a good result with implants alone, no matter how large they are. You will in this case require a breast lift, with or without implants (depending on what volume you wish to achieve).

The tricky judgement call arises when the nipple AND/OR the bulk of the breast tissue lies AT around the breast fold level. These “borderline” situations demand one other important question to be answered: What size or volume increase are you looking for?

In cases of borderline droopiness, the patient MAY be able to get away with a reasonable result with an implant alone; however, the caveat is that implant must be of a large enough size that will allow the loose lower skin to be filled. The result of such an approach may give a satisfactory result, but it certainly won’t be as “perky” a result when compared with a lift or the result gained with implants in a non-droopy patient. If you are a borderline case and only looking for a small increase in size/volume, then a lift is often advised.

The trade off with a lift (with or without implants) is, of course, the inevitable scarring. A breast lift (or mastopexy) involves elevating the nipple areolar complex to a higher, more desirable position, removal of lax breast tissue, and tightening of the breast skin envelope. This will result in circular scar around the nipple areola, a vertical scar down the centre of the breast, and often a horizontal scar across the breast fold. This scarring in most patient will fade with time and become quite inconspicuous.

So there you have it. A quick and simple analytical process that enables you a fairly good idea to see if you are a good candidate for breast implants alone or if you require a breast lift. Remember, nothing beats a comprehensive assessment with an experienced Breast Implants Surgeon Brisbane. Know what kind of result you are looking for in terms of SHAPE and SIZE. Discuss these with your surgeon to see if they are achievable. There are many other factors (such as skin quality, breast tissue composition, tubular deformity etc) that play a part in the decision making process, and your pre-existing anatomy may also limit what can be achieved.

The left side however, with the nipple above the fold, but with the bulk of the breast tissue starting to fall below the fold, will either need an implant large enough to fill out the lax lower breast tissue or a smaller implant together with a Breast Lift in Brisbane.

Why is Revision Rhinoplasty Tougher?

Revision rhinoplasty, also commonly referred to as secondary rhinoplasty, is any time the nose has surgery performed on it after the first surgery. Revision rhinoplasty is more often than not more difficult to perform than a primary (first time) procedure. It takes more time to complete, will often require materials gained from other parts of the body, and requires more knowledge, experience, and technical skills on the surgeon’s side.


So why is secondary rhinoplasty much tougher to perform? Well, to begin with, the anatomy is no longer pristine, but rather, there will often be an abundance of thick, hard scar tissue from the previous operation. Scar tissue obscures the natural tissue planes that separate the various cartilages, bone, and soft tissue within the nose. Precise and careful dissection is often painstakingly necessary to separate the scar tissue from normal structures of the nose.
Defining the normal existing anatomy of the nose is only the beginning and this alone can take hours. Once the existing anatomy of the nose has been clearly defined, the surgeon then has to assess what’s present, what’s absent, what’s normal, and what’s abnormal. This assessment is followed by correlation with the current clinical picture and what is the desired final outcome. To achieve this desired result, the surgeon then has to begin formulating a plan to “re-build” the nose.

The nose is like a tent, supported by its poles. Underneath the skin of the nose, there are inter-related cartilage structures that give shape to the nose. More importantly, these cartilages also keep the nostrils open and allows a patent airway to breathe through. Not uncommonly, a revision rhinoplasty case will present a nose that lacks structural support. Imagine a tent which has weak tent poles. A nose like this will lack strength and is at risk of collapse. One of the reasons why this type of nose may lack support may be due to over-removal of tissue from the previous operation.
To restore structural support to the nose, one has to find material (usually cartilage) to build and also support the construct of the nose. The cartilage that is used is often taken from the septum (the structure that runs down the middle of the nose), the ear, or the rib. “Harvesting” this cartilage material is another factor which adds to the complexity and time required for a revision rhinoplasty. It also adds to another scar on the patient.

At the outset at any rhinoplasty, one has to devise a plan together with the patient that aims to achieve the ideal outcome through one, first, and final, procedure. That been said, as with any other surgery, there is always the chance that the outcome achieved may not be 100% in line with what the patient expects. Complications can occur, and revision surgery is always a possibility. With nose surgery, both surgeon and patient need to be extra patient before they evaluate the final result. Swelling in the nose can persist for many months after surgery, especially in men and in thick skinned people.
One must be careful not to rush into performing a revision before the final shape of the nose is realised. The shape and function of the nose will continue to change and often improve as swelling subsides and the scar mature. Regular follow up between the patient and surgeon is therefore crucial to ensure that the patient is progressing in the right direction.

In any cosmetic surgery consultation, one of the most important things to establish from the very onset are the patient’s wishes and goals.
In Breast Augmentation, this means that the surgeon must understand and determine the kind of result the patient is hoping for, in terms of breast shape and size. After surgical goals have been established, it then becomes easier for both surgeon and patient to work through the type and size of implant, pocket placement, and surgical technique to be applied in order to achieve the best result.
To make it easier in my consultations, I like to think of the goal (or final result) from breast augmentation as broken into four categories. Most patients will fall into one of these goals:
  1. “You’ve got breasts”
    This is a small and subtle increase in size. You do not want anyone to know that you have had breast augmentation, and people around you may not notice the change.
  2. “Nice breasts”
    This is a natural result, with a gentle slopping upper pole, and a round lower pole. There is often little hint to others that your breasts have been surgically enhanced.
  3. “Are they real?”
    The breasts are large, but may still be accepted as just falling within what nature can endow. There is fullness and roundness in the upper pole and sides.
  4. “They are not real”
    Size rules here – as big as can be done – the breasts are disproportionally large compared to your body. Patients in this category wish to have the “fake” look.
Remember, for each patient, there is a safe range of implant shapes and sizes that can be used to augment the breasts. Once you venture beyond this safe range, the risks of future complications such as displacement and capsular contracture dramatically increases. The final result gained from breast augmentation is a very subjective matter. The eventual size of implant used is often chosen by the patient; however, it is the surgeon’s role to guide the patient in making this decision based on the patients wishes and their breast anatomy.
